Create a memorial in honor or memory of your loved one:

Creating a memorial is a meaningful way to honor the life and legacy of someone special while supporting healthcare in our community. Families may choose to direct memorial gifts to the Henry County Health Center Foundation or Park Place Long-Term Care in lieu of flowers or other expressions of sympathy.


For many families, Park Place holds a special place in their hearts, having provided compassionate care, comfort, and support during a loved one's final chapter of life. Because of this meaningful connection, many families choose to establish memorial gifts that benefit Park Place and help continue its mission of caring for residents and families.

Family Notification & Gift Acknowledgments

When making a tribute gift, donors will provide the name of the individual they are honoring or remembering, along with permission on who to notify in the loved one's family. The Foundation will send a thoughtful acknowledgment informing the family that a gift has been made in tribute to their loved one. Gift amounts will remain confidential.

Making a memorial gift is a meaningful way to honor the life and legacy of someone special. Gifts may be directed to an established memorial supporting the Henry County Health Center Foundation or Park Place Long-Term Care, helping continue compassionate care and healthcare services that benefit our community.


If you would like, the Foundation can notify a family member or loved one that you made a gift to the memorial. Gift amounts will remain confidential.

  • Retirement Plans

    Naming the Henry County Health Center Foundation as a beneficiary of your IRA, 401(k), or other retirement account is a simple and impactful way to support the future of healthcare in our community. You can designate a portion of your retirement assets to the Foundation. For donors age 70½ and older, a Qualified Charitable Distribution (QCD) may also be an option. By setting up a gift annuity, you agree to exchange your irrevocable transfer of cash or securities; you receive an immediate charitable tax deduction and a dependable income stream. When the annuity ends, the remaining funds support the Foundation and help advance healthcare in our community.

    This is another popular planned giving option because it allows individuals to support a charitable cause, while also creating a reliable source of income for themselves or a loved one. 


     

  • Charitable Remainder Unitrust

    A Charitable Remainder Unitrust can provide income for you or your loved ones while also creating a future gift for the Henry County Health Center Foundation. Assets are placed into a trust, and the trust makes annual payments based on a percentage of its value. Because the trust is revalued each year, payments may increase or decrease over time. You may also qualify for a charitable income tax deduction when the trust is established. After the trust term ends, the remaining assets support the Foundation.
